Job Description:
A company is looking for a Transition Specialist to support the
coordination of member transitions in the community.
Key Responsibilities
Support care coordination teams in developing effective transition
plans for members
Assist with member assessments and the development of transition or
service plans
Educate members and families about services, requirements, and
limitations during the transition process
Required Qualifications
Bachelor's degree
2 - 4 years of related experience
Equivalent experience may be considered
Ability to track and maintain transition metrics
Experience in drafting educational materials for members
